Retirement Notification Form
Once you’ve made the decision to retire and you know your retirement date, then you may submit your Retirement Notification Form to HR notifying LPPS of your intent to retire. This form is for LPPS use only. This form does not notify your retirement system of your intent to retire. It is your responsibility to submit the appropriate Application for Retirement to your retirement system, and submit the LPPS Retirement Notification form to LPPS Retirement Office for processing. Submitting this form permits LPPS to process your final pay and allows your principal/supervisor to hire someone for your position. This form will also allow you to state that you would like to continue your health/life/dental insurance coverage after retirement.
Remember, your retirement date is the day after your last day of employment, even if that day falls on a holiday or weekend. For instance, if your last day of work is Friday, December 20, 2013, then your retirement date will be Saturday, December 21, 2013.
Keep in mind, when you retire or enter DROP, we will pay up to 25 days Severance Pay, but we must receive the request for payment of those days in writing. Please use the Severance Pay Form provided here to request payment for Severance Pay.
